Medication Errors

The term ?medication error? covers a whole manner of sins. What is a medication error? Perhaps it is better to explain by providing examples of medication errors.

Example A ? your physician is aware you are allergic to a medication but prescribes it anyway;

Example B ? Other medications you are currently taking are contra-indications to a new medication your physician just prescribed, but she doesn?t read the medication package insert and now you are taking medications which may prevent the others from being effective, or worse, their combination may cause injury;

Example C ? Medicine A can only be taken by people who can walk or are otherwise mobile, and physician prescribes it for his double amputee patient who cannot get out of bed;

Example D ? Pharmacy receives a prescription for medication A but fills your bottle with medication B;

Example E ? Pharmacy fills correct prescription but prints wrong dosing directions so you are now taking the medication 9 times a day, rather than 3 times per day ordered by your doctor. An overdose occurs causing injury.

There are almost too many examples of medication errors to list. And many players in the health care field can cause or fail to prevent medication errors, from physicians, to nurses and pharmacists. Medication errors are such a big deal in health care today that the Federal Drug Administration (FDA) has been researching errors these errors while attempting to create and implement programs and rules which would prevent such errors. How is the FDA doing at minimizing these errors and the resulting needless injuries and deaths? Well, it must be a difficult challenge because the FDA admits ?Improving patient safety continues to be a challenge.? So what can be done to prevent medication errors?

  1. Keep an active record of all medications you are taking. Write it down and provide copies to all physicians.
  2. Research any medication allergies. How do you know you are allergic? What is the generic name of the drug to which your are allergic ? know it all.
  3. Tell your pharmacist about all medications you are taking. He may only have records of medications dispensed at his pharmacy ? so give them notice of all medications.
  4. Tell your family and provide them with a list of your medications. It is likely that in an emergency you will not be able to speak for yourself.
  5. Speak to your physician ? ask why she is prescribing each medication? What are the side effects? Have you been on this medication before? Keep a record.
  6. Unsure about dosage? Unsure about combination with other medications? CALL YOUR PHYSICIAN? Call, and write down the name of who you spoke with and what they said. KEEP NOTES.

Pretty similar list to ours ? sadly, both lists are created for consumers and patients. And it is important to note that it is not your duty to prevent all medication errors. It is the duty of your healthcare provider to do so, but if they fail, your diligence may be the difference between another preventable injury or death from a medication error and a close call.

Reiki Self Attunement is Best

The process of become a Reiki Master involves going through 4 different Reiki Levels. To move up a level requires you to be Reiki Attuned to the next Reiki Level.

Having done both in-person Reiki level attunements and much study about energy medicine, I can assure you that books are equally as powerful. Pain Killer Addiction - Don\’t Be Overwhelmed - Get Help Now

There are many side effects and adverse reactions that can occur with the use of opioids as pain killers. Patients can innocently start taking pain killers after a moderate injury or because of a severe injury in an automobile accident, fall or for post surgical pain. More than 10% of high school seniors have started taking Vicodin for reasons other than reducing pain.

If you are addicted to pain killers or other drugs or think you may be, you can start working to increase the body\’s endorphin production naturally; some ways are laughing, touching, massage, acupuncture, acupressure, walking, anything that makes you feel good that\’s natural. More than 415,000 people received treatment for pain killer abuse or addiction this past year. Treatment options for pain killer addiction include: medications, such as methadone and LAAM (levo-alpha-acetyl-methadol), and behavioral counseling; usually, the patient is medically detoxified before any treatment approach is begun.

When you\’re addicted physically to a drug, like pain killers or alcohol, etc., it\’s because you\’ve suppressed or shut down your body\’s production of endorphins, which are natural opiate pain killers; when this happens you start craving the drug that you replaced the endorphins with whether it\’s alcohol, any of a number of drugs or pain killers. Pain killer addiction includes: opiate dependency, opiate addiction, narcotic dependency, narcotic addiction, and pain killer dependency or painkiller dependency. Chronic pain affects one out of three or four adults; millions of people suffer from severe disabling pain.

Physical dependence on a drug suggests that sudden stopping of the drug may result in negative consequences. Opioids used as the doctor has prescribed are supposedly not dangerous according to some well-established medical groups; but if this is the case, why are so many people addicted to them? If you think you are addicted and want to get off pain killers or other drugs, it\’s best to get detoxified as fast as you can and then go through some type of rehabilitation; it\’s important to have others to lean on and learn from and offer support to you.

Taking the time to spend in a treatment center, detoxing, is of the utmost priority. If you don\’t have insurance, check with your local mental health agencies to see what is available that\’s free. It\’s important to get help and not to try getting off pain killers on your own.

All other demands of children, a job, school, or any other responsibilities may make inpatient treatment seem like an intrusion but it\’s not. If you can\’t do an in-patient rehab, find out how you can do outpatient rehab and pay for it under your insurance plan; check your insurance policy to see if it\’s covered. It\’s important to go through rehab following your detox stay: make it a part of your plan of action.

Recent studies have shown that six out of 10 people in the United States have some form of debt. Exhibitions, Tradeshows And Events - The Single Most Important Ingredient To Your Success

There is ONE central idea, one key concept that I want you to understand. If you get this right, your show (exhibition or event) will reward you with customers buying what you have to sell. If you DON\’T get this right, you\’ll be paying a fortune for next to nothing, your competitors will eat you up, and your whole exhibiting experience will be very, very unpleasant.

Set Objectives!

There are many things that drive success to your show, but none more so than setting objectives. Without objectives you are down and out before you even get in the ring. Yes they may not sound exciting or urgent but believe me they are incredibly important.

The vast majority of exhibitors do not set objectives - they book a show because their competitor does; they don?t know who really goes; they don?t know what image to portray or products to display; they don?t know, don?t know, don?t know!

Paying good money and then turning up at your event without objectives is like going to war without knowing why and of course you\’ll never know if you?ve won or lost!

Objectives are not just about what you want to get out of the exhibition or show, they include how you are going to get the results - for example - How are you going to measure your success? Or how will you follow up leads?

Up to 80% of your competition at any exhibition, trade show, trade fair, conference or event will be what we call ?logistic exhibitors? - not ?strategic exhibitors?.

What?s the difference?

Most exhibitors focus on the logistics of the event (for example, ensuring the stand is bought, that the staff are booked, that the transport is laid on, that the products are ready, that the hotels are booked etc) and making sure they don?t miss that critical deadline.

Strategic exhibitors of course focus on all of the above but realise that there are equally (if not more) important tasks that whilst not seeming urgent right now are absolutely critical to the show?s success; these include setting objectives, how to measure their results and how they will follow up leads to name but a few.

So why aren?t more exhibitors strategic? There are many reasons, but in most cases logistic exhibitors fall into one of two categories:

1. Small Business ? Who don?t have the time, the knowledge, resources or know how. If this is you, then you need to spend time doing all of the above or think very seriously about not attending at all - It could save you a fortune and a very big headache!

2. Big Business ? With fairly typical communication problems. This often develops when the senior management doesn?t communicate what it expects to the organiser (that?s probably you!), who in turn is busy trying to achieve what they think the senior management wants to hear ? the result mixed messages and mixed results. If this is you, then you need to start knocking some heads together or at the very least send an email to tie up the loose ends.

Become a Strategic Exhibitor?

Becoming a strategic exhibitor is easy; you just need to do two things to make it happen. Firstly, with the best will in the world, your show will not become a success just by you reading our e-course ? you must implement the information and make it happen! Secondly, become a strategic exhibitor, stay focused, follow our system and do everything that is required to make your show a success ? not a logistic exhibitor who just does the obvious.

Do not under-estimate these two simple steps - by you doing just these two things will have a serious impact on the success of your show.

Setting Objectives?

Your first question should be ?Why are we going??

If you?re in a business-to-business selling situation then you might think you should be exhibiting and your reasons might be to generate leads, find new customers, or maintain or improve your current customers perceptions of you.

Whilst this is fine in general, you need to get to the very heart of why you (or your organisation) are going and what would ?success? actually look like? What would have to happen that would make the event a great one?

By sitting down early and getting these objectives on paper now, will ensure that you (and your colleagues) follow a course to exhibiting success.

Bikers can enjoy one of four Biking Heritage Tours of the Northern Neck, while hikers can partake in an eco-tour of the 733-acre Belle Isle State Park or the 6,000-acre Rappahannock River Valley National Wildlife Refuge.

Effective Teen Parenting Is Challenging

We all know that the teen years can be viewed as a journey from childhood to adulthood, but consider that teen parenting can be seen as a similar journey. In the growing teen, they are dealing with getting older, accepting more responsibilities that they never had before, raging hormonal changes, discovering their own identity, and a struggle for independence. All of these things make teen parenting a challenge, since teens can go from one extreme to the other in the blink of an eye.

One more teen stress that needs to be considered is the inevitable want to fit in, to be accepted by their friends and peers. This force is incredibly strong, nobody wants to be unaccepted or shunned, so this is also something to consider when you are doing teen parenting.

Why are these facts laid out at the beginning? Because in countless studies of the conversations that take place in counseling centers across the nation, the phrases commonly heard by the teens regarding their parents are things like “you don\’t understand” and “leave me alone”. It is true, as parents we tend to forget what our teen years were like just a few short years ago, where we had exactly the same issues to be dealing and struggling with, and understanding what is going on in the teen mind and what is important to today\’s teen can go a long ways to having a better relationship and more open communications with your teen.

Teen parenting is also a struggle on walking a very fine line between enforcing rules and limitations without negatively impacting the teen\’s sense of freedom. This is indeed a fine line and parents will inevitably cross that line sometimes, which can turn their once-cuddly little baby into a fierce alien from another planet. Teen parenting is not an easy task, and nobody will advocate just giving the teen free rein to do what they wish, since they seem to know it all anyway. Rather, effective teen parenting skills include patience, creativity, and courage to set and lovingly enforce the rules and boundaries that need to exist in a household.

This fine line should be seen as a challenge that you can overcome in order to teach your teen values and to know right from wrong. In the process of teen parenting, take the necessary steps to guide your child into adulthood by instilling social values, setting and achieving goals, which will aid their emerging independence and set a line for keeping things on the right track for the future. By all means, do NOT view this as a cumbersome task that you are just not up to. Teens will sense this and will stretch your patience to the breaking point. Maintain focus!

In teen parenting, it is critically important to keep a good solid and loving relationship in place, with a wide-open door of communications. While you are the parent, you can also be the best friend, someone they can feel free to confide in and to ask advice of. While not a topic to dwell on, let them know that you were once in their shoes, and you DO understand what they are going through and what they are struggling with. As strange as it may sound, that fact may come as a surprise to your teen, who might view you as never having been in your teens!
